Where the structure is sound and only the heights are off, the work is closer to careful adjustment than to reconstruction, and it falls under pier and beam repair. Where framing has to come out, expect the job to grow a little once the finishes are off, which is why the quote carries an estimated schedule.

The number of supports that need work, how much framing has to be replaced, and how much room there is to move around down there. Is a vapor barrier worth putting down?

In most cases yes. A sheet over the soil stops ground moisture evaporating up into the framing, which is the main reason crawl spaces stay damp. Can I jack up the floor myself?

Please do not. The house is held on a small number of loaded points, and taking weight off the wrong one can shift the structure suddenly with you underneath it. Lifting too far in one go splits framing and cracks walls above. It is safe to look, take photos and note where the floor dips, and leave the jacking to the crew.

Will fixing the crawl space stop the musty smell?

Usually, once the source of the moisture is gone and the space can dry out. A smell that persists after the ground is covered and the water routed away is worth investigating further, because it often means a leak or condensation rather than ground damp.
